1. Contact Information: Provide your name, mailing address, telephone number, email address, and artist website or social media (if applicable).

2. Exhibition Selection: Indicate which Collective Series exhibition you wish to participate in.

3. Artwork Information: For each artwork submitted, provide the title, medium, dimensions (height × width × depth), year completed, and sale price.

4. Creative Reflection: Share a brief reflection (50–150 words) describing what inspired your interpretation of the selected moon theme, the ideas or experiences that influenced your work, or the story you hope to share with visitors.

5. Images: Submit one high-quality image of each artwork. Images should be clear, well-lit, and accurately represent the artwork.

6. Artist Sales Information:All artwork sales are processed through Moose River Studios. A 5% Artist Sales Service Fee is deducted from each sale to cover payment processing, administration, inventory management, sales tracking, artist settlements, and customer service.

7. Participation Fee: Participation fees are charged per artwork and are payable upon acceptance into the exhibition.​

8. Artist Agreement: Confirm that the submitted artwork is your original work, that you agree to the exhibition schedule, delivery and pickup dates, and that you understand the exhibition guidelines, Artist Sales Service, and gallery policies.
